Vaccination Requirements for School Entry

In 2016, vaccination requirements for school entry have changed.

Children entering preschool, kindergarten (or transitional-Kindergarten) and seventh grade (see “Ages 7-17” section) will have to be fully vaccinated for their ages.

The only valid new exemption starting in 2016 is a medical exemption specifying why vaccines are not safe for this child, which vaccines are included and the duration of this exemption.

Personal Beliefs Exemptions which were filed with a school prior to 1/1/2016 will allow school entry for continuing preschoolers, children entering grades one through six and children entering grades eight through twelve. These PBEs are valid when changing schools or changing districts.

The new law (SB277) applies to private and public schools equally.

Parents (our patients) whose children cannot be vaccinated for medical reasons may fill out a questionnaire we’ll provide stating why they think their children qualify for a medical exemption from vaccines. We will evaluate these needs and write the exemption letter if medically indicated.

We can only care for patients within our practice and we will not be seeing new patients seeking a one-time consultation for medical exemptions.

(4 doses OK if one was given on or after 4th birthday)

(3 doses OK if one was given on or after 4th birthday)

Hepatitis B—3 doses

Measles, Mumps, and Rubella (MMR)—2 doses
(Both given on or after 1st birthday)

Varicella (Chickenpox)—1 dose

(2 doses required at 7th grade)

(Admission at ages 7-12 years need 1 dose; ages 13-17 years need 2 doses)

Tetanus, Diphtheria,and Pertussis (Tdap) —1 dose at 7th grade or out-of-state transfer admission at 8th–12thgrades
(1 dose on or after the 7th birthday)
